Manage client forms

Create intake and consent forms, link them to services, and collect completed responses online or on site.

In this guide

Create and organise a form

  1. Open Forms to create a form from scratch or start from a standard template.

  2. Preview a saved form before sharing it to confirm every heading, question label, option and required marker. Use More actions to copy the reusable public link or download its QR code as a PNG without creating an appointment. Creating, editing, duplicating and archiving templates requires form-management permission; view-only roles can still preview and share active forms.

  3. Add required fields only where the answer is needed before the business can proceed safely.

  4. Use section headings to group longer intake or consent forms into clear parts.

  5. Use conditional visibility when a question should only appear after a previous answer, such as showing allergy details only when the client has selected an allergy option.

Set completion and delivery rules

  1. Choose a completion rule: Can be completed later keeps booking non-blocking, while Needed before treatment flags the appointment until the form is submitted.

  2. Link forms to the services that need them. When those services are booked, Schedmo creates customer-specific form requests tied to the client, appointment, and service.

  3. Use delivery settings on the form template to decide whether the form can be sent automatically by email or SMS after booking.

  4. Use Settings > Forms automation to enable or disable business-wide automatic email and SMS delivery.

  5. SMS form delivery is only queued when the business has an active Schedmo subscription and valid billing details on file.

Send and collect responses

  1. From an appointment, open the Forms tab to add forms manually, send email or SMS, copy the secure customer link, or show the QR code so the client can complete it on a phone or tablet.

  2. If a form link has expired, create a new link from the appointment or client form row before sending, copying, or showing the QR code.

  3. From a client profile, use Forms to add a standalone form and collect it the same way, even when it is not attached to an appointment. An anonymous public form submission creates a provisional customer record using the supplied name, without changing or claiming an existing customer profile. Its signed response is labelled as an unverified identity. Verify the person with your usual process before treating that response as evidence for an existing client; issue a customer-specific request from the verified client record when needed.

Review responses and portal access

  1. Use Forms > Responses to filter completed, incomplete, expired, needed-before-treatment, email-sent, and SMS-sent form requests.

  2. Completed responses stay linked to the client record, appointment when relevant, form version snapshot, and signed response artifact.

  3. When forms are enabled in Client portal settings, signed-in clients can open pending requests from Forms, complete them securely, and review completed status without using a staff-workspace account.
